Please print and sign the Acknowledgement of Receipt of Privacy Practices (we can also provide a copy for you to sign when you arrive).

Items to bring to your first appointment at Hartley Bridge TMJ & Sleep Center

• Driver's License or Photo ID (required by most insurance companies)
• Your Physician and Pharmacy contact information
• Any oral appliances you are currently using
• A copy of your most recent sleep study or the sleep testing facility contact information (for sleep apnea patients)
